What to Provide for Tax Preparation

Existing Clients

If any of the following information has changed since the previous year, please provide us with your most updated:

  • Address, Telephone Number(s), Email Address(es), Occupation(s), Bank account and Routing number or voided check for direct deposit of your refund.
  • For New Dependents: Full Name, Birth Date, Social Security Number or copy of SS Card (or Social Security Number verification letters, or Individual Taxpayer Identification Number assignment letters).
  • Identity Protection PIN (IP PIN), if the IRS has issued one to you.

NOTE: *Your return WILL BE rejected if this PIN is not included in your tax return.

New Clients

Please provide the following information:

  • Photo ID
  • Social Security Number or copy of SS Card (or Social Security Number verification letters, or Individual Taxpayer Identification Number assignment letters) for you, your spouse, and any dependents
  • Birth dates for you, your spouse, and dependents on the tax return
  • Bank account and routing number or a voided check for direct deposit of your refund
  • Prior year tax return
  • Identity Protection PIN (IP PIN), if the IRS has issued one to you. NOTE: *Your return WILL BE rejected if this PIN is not included in your tax return.
  • Please download & complete a New Client Interview Form

Employment Income

W-2 form(s) for ALL jobs during the tax year. (All employers are obligated to provide you with this form by January 31st by mail and/or electronic format).

Self-Employment, Business, Contract Work Income

Have Self-Employment Income?

The IRS holds each taxpayer responsible for accurately recording their income and expenses and providing this information for accurate tax preparation. We cannot use your previous year's expenses, nor do we know what your expenses have been during this tax year unless we have provided full bookkeeping services.


Why are expenses important? Business expenses can help offset some or all of your income with deductions.

  • Please download, complete and provide your SELF-EMPLOYMENT EXPENSE SHEET.

Total amounts only; Keep receipts, canceled checks, credit card statements, etc. for your records.

  • 1099-NEC(S) for ALL work during the tax year.
  • Total amount of income not reported on 1099-NEC form
  • Total amount of estimated tax payments paid during the tax year

Other Income

  • W-2G: Lottery/gambling winnings
  • 1099-K: Payments received from credit, debit or stored value cards such as gift cards, or payment apps, and/or online marketplaces for goods or services (third party settlement organizations or TPSOs)
  • 1099-MISC: Rental, Prize Money, Other
  • 1099-R: Retirement
  • 1099-SSA: Social Security Benefits
  • 1099-INT: Interest Income
  • 1099-DIV: Dividend Income
  • 1099-A & 1099-C: Debt Cancellation
  • 1099-G: State Refunds & Unemployment benefits. (Download from State website)
  • K-1 Schedule: from S-Corp, Partnership, Trust, or Estate 

Other Sources of Income

Income or loss from the sale of stocks, bonds, mutual funds, Bitcoin, Cryptocurrency, or digital asset transactions:

  • 1099-B (Investment Brokerage Statement)
  • and/or other statement(s) providing description of sales and costs of investments.


Sale, gift, or disposal of any Bitcoin, Cryptocurrency, or digital asset:

  • Transactions statement(s) providing description of sales and costs of investments.


Sale of home:

  • 1099-S 
  • "ALTA" Closing Statement
  • Improvements to the home over the entire time lived in the house which maintained or added to its value.
  • Anything else done to the home 6 months before the home's sale, including cosmetics.
  • Expenses related to the sale of the home, including advertising costs. 
  • 1098 Mortgage Interest Statement
  • Property Tax Bill


Purchase of home:

  • "ALTA" Closing Statement
  • Form 1098 Mortgage Interest Statement
  • Property Tax Bill


Income or loss from rental property:

  • Download, complete and provide RENTAL EXPENSE SHEET (Report each property separately using one expense sheet per property.) Total amounts only; Keep receipts, canceled checks, credit card statements, etc. for your records.
  • Form 1098 Mortgage Interest Statement
  • Property Tax Bill


Alimony paid or received *Only under certain circumstances. Speak to us for more information.


Any & all sources of other income received

Expenses

To claim additional tax deductions to reduce your taxable income, provide the following documentation if you have any of the following expenses:


  • Retirement contributions, including a 401(k) or IRA
  • State and local taxes paid
  • Mortgage statements and property tax bills, if you are a homeowner (Form 1098)
  • College tuition (Form 1098-T) and student loan statements (Form 1098-E)
  • Childcare expenses: Include total amount paid per child, provider’s name, address, and federal tax ID number (either their Social Security Number or Employment Identification Number)
  • Charitable donations (Cash or Non-Cash)
  • Medical and dental bills 
  • Educators/Teachers: Provide supplies used as an Educator/Teacher.


Total amounts only; Keep receipts, canceled checks, credit card statements, etc. for your records.

Other Important Tax Documents or Information

  • 1098-T: Tuition, scholarships/fellowships
  • 1098-E: Education Loan Interest
  • Private Mortgage: Name, Address, SS#, Amortization Schedule
  • Boat or RV as Second Residence: Bank Name, Address, Federal ID#, Interest Paid – no 1098 will be issued by the Bank


HEALTH INSURANCE MARKETPLACE during ANY TIME of the year?

  • 1095-A: If you had health insurance through the Health Insurance Marketplace during ANY time of the year.

To get your 1095-A form:
Your 1095-A should come by mail by mid-February. It may be in your Marketplace account anytime from mid-January to February 1. Log into your account, select your 2024 application, and then select “Tax Forms” to get your form.  Or contact 1-800-318-2596 to speak to a Healthcare Marketplace representative.


NOTE: *Your return WILL BE rejected if this form is not included in your tax return.

Correspondence & Letters from IRS and/or any State Tax Agency

All Documents or Letters received from the IRS, and state tax agency, or anything that says “IMPORTANT TAX DOCUMENT”
