What to Provide for Tax Preparation

Existing Clients

Only send updated personal information if something changed since last year:


  • Name, address, phone, email, or occupation
  • Bank account and routing number for direct deposit, if refund is expected
  • New dependents: full name, date of birth, Social Security number/ITIN documentation
  • IRS Identity Protection PIN (IP PIN), if issued. (If the IRS has issued you an IP PIN, you must provide it. Missing or incorrect IP PINs can cause an e-file rejection or delay processing of a paper return.)

New Clients

Please provide:

  • Photo ID
  • Social Security Number or ITIN documentation for you, your spouse, and any dependents
  • Birth dates for everyone on the tax return
  • Bank account and routing number for direct deposit, if refund is expected
  • Prior year tax return
  • Identity Protection PIN (IP PIN), if the IRS has issued one to you. (If the IRS has issued you an IP PIN, you must provide it. Missing or incorrect IP PINs can cause an e-file rejection or delay processing of a paper return.)
  • Completed New Client Interview Form

Income Documents

Provide all tax forms you received, including:

  • W-2 wages
  • 1099-NEC nonemployee/contract income
  • 1099-K payments for goods or services
  • 1099-MISC other income, rents, prizes, etc.
  • 1099-INT interest
  • 1099-DIV dividends
  • 1099-R retirement income
  • 1099-SSA Social Security benefits
  • 1099-G unemployment or state tax refund 
  • W-2G gambling winnings
  • K-1 from partnership, S corporation, trust, or estate
  • 1099-B brokerage sales
  • 1099-A or 1099-C debt cancellation
     

Also include any taxable income not reported on a tax form.

Self-Employment or Business Income

If you are self-employed, a contractor, or a business owner, provide:

  • Completed Self-Employment Expense Sheet
  • All 1099-NEC and 1099-K forms
  • Total income not reported on a form
  • Total estimated tax payments made during the year
     

Provide totals by category. Keep your receipts and records for your files.

Rental Property

For each property, provide:

  • Completed Rental Expense Sheet. Use a separate expense sheet for each property.
  • Mortgage interest statement (Form 1098)
  • Property tax bill
  • Income and expense totals for that property

Sale or Purchase of Real Estate

If you sold a home or other property:


  • 1099-S, if issued
  • Closing statement (ALTA or similar)
  • Major improvements made to the property
  • Selling expenses
  • Mortgage interest statement
  • Property tax bill
     

If you purchased a home:

  • Closing statement
  • Mortgage interest statement
  • Property tax bill

Investments and Digital Assets

If you sold stocks, mutual funds, cryptocurrency, or other digital assets, provide:

  • 1099-B or brokerage statements
  • Transaction reports showing purchase price, sale price, and dates

Deductions and Credits

Provide documents for any that apply:

  • IRA or other retirement contributions not already shown on tax forms
  • Mortgage interest and property taxes
  • College tuition (1098-T)
  • Student loan interest (1098-E)
  • Childcare expenses, including provider name, address, and tax ID
  • Charitable donations
  • Educator expenses 
  • Medical expenses, if significant
  • State and local taxes paid, if applicable

Health Insurance Marketplace

If anyone on your return had Marketplace insurance at any time during the year, provide:

  • Form 1095-A
     

We need this form to reconcile the Premium Tax Credit and prepare your return correctly.


To get your 1095-A form:
Your 1095-A should come by mail by mid-February. It may be in your Marketplace account anytime from mid-January to February 1. Log into your account, select your 2025 application, and then select “Tax Forms” to get your form.  Or contact 1-800-318-2596 to speak to a Healthcare Marketplace representative.

NOTE: *Your return MAY BE rejected if this form is not included in your tax return.

IRS or State Tax Notices

Provide:

  • Any IRS letters
  • Any state tax notices
  • Any document marked “Important Tax Document”
